According to the New Jersey State Mandates, each candidate who wishes to participate in athletics, MUST be examined by their primary health care provider within 365 days prior to tryouts. The NJ State Annual Athletic Pre-Participation Physical Evaluation Form MUST be used for the physical exam.

Any participant who is medically excused by a nurse, doctor or trainer, cannot participate in school team practices or games until cleared by a doctor. Furthermore, any student who is medically excused from participation in Physical Education cannot participate that day in the school team practice, or game.

EVERY SPORT, EVERY SEASON, YOU MUST HAND IN PAPERWORK TO THE NURSE 

To check the date of your child's last physical exam on file, log onto Genesis and click the documents tab, then click "physical exam status notification."
